Learning Logic

Learning Logic is a highly-interactive, computerized mathematics program currently used to teach Algebra I (Pre-algebra and Algebra II on a limited basis). It presents lessons which are self-paced, is easily adapted to individual students, and generates homework and tests which are graded by the computer and are based on the material covered by each individual student.

    Not free
The web page doesn't really say, but after emailing the given contact on there, this program is not free. One-time charge of $19,000 per server plus an annual $25 charge per student. I doubt you'll ever see any source code either.
