What is HL7?
You might want to explain what HL7 is. Is it some sort of API for Medical apps? (searching on Google gives some hits for that). It doesn't say on your site either.

Isn't this exactly for the SVR4ish /opt is for?
My SGI puts apps into a fairly reasonable heirarchy in /opt. As long as there are tools to deal with the resultant gigantic PATH in sensible ways, I'd prefer that to having to figure out which of the 300 binaries in /usr/local/bin are actually related to imagemagick or netpbm (at least netpbm follows a vague naming convention).
My personal objection to the original description is the (apparent) reliance on a particular type of GUI/desktop to do the dirty work. Of the machines I run, exactly none have X consoles.
